Case Manager – RN
Hospice of Knox County (HKC) is CHAP accredited and committed to providing specialized end-of-life care to patients of all ages. HKC has an opportunity for a full-time Case Manager who has a desire to improve the quality of life for terminally ill patients. The Case Manager plans, organizes and directs hospice care and is experienced in nursing. The successful candidate will be responsible for developing and implementing the comprehensive interdisciplinary care plan for terminally ill patients, and the provision of palliative nursing care, including ongoing support to the patient’s family.
Position requirements include RN and current licensure to practice nursing in Ohio; degree from of an accredited school of nursing, and three years professional nursing experience in home health and institutional setting or equivalent combination of education and experience. Bachelor’s degree, with (1) year of hospice care preferred. Must be able to work a flexible schedule; valid Ohio driver’s license with current automobile insurance and reliable transportation for travel while on Hospice business.

Music Therapist
30 hours per week
Founded in 1983, Hospice of Knox County (HKC) has earned a solid reputation among not-for-profit hospice and palliative care organizations. HKC is CHAP certified and committed to providing specialized end-of-life care to patients of all ages. HKC has an opportunity for a Board Certified Music Therapist who has a desire to improve the quality of life for patients with a life limiting illness. The successful candidate will be responsible for providing music therapy as part of psychosocial and physical treatment of patients and families to influence behavioral and psychological changes.
Position requirements include a bachelor’s degree in music therapy; music therapy board certification (MT-BC) by CBMT; 2 years related experience; hospice experience preferred; excellent communications skills; experience with Microsoft Office Suite programs.
Located in Mount Vernon, Ohio, HKC is 40 minutes northeast of Columbus, the state’s capital city. Mount Vernon is a quiet community that offers the convenience of a small town with easy access to large cities.
